Pain radiating to the right upper quadrant and right shoulder, nausea and vomiting, fever and general weakness are common. On examination, increased tenderness in the right upper quadrant during deep breathing, called the “Murphy's sign,” supports the diagnosis. If the stone also obstructs the bile duct, jaundice and darkening of the urine color may be added. The condition may be more insidious, especially in advanced age and in the presence of diabetes, so clinical suspicion is important.

A balanced diet, regular physical activity, and avoiding rapid weight loss are key approaches, as extremely low-calorie diets and rapid weight loss after bariatric surgery can trigger stone formation. A dietary pattern rich in fiber, rich in fruits and vegetables, and low in saturated fat (e.g. Mediterranean diet) is recommended. Routine surgery may not be necessary in individuals with gallstones but without symptoms; the decision should be individualized based on symptoms, stone characteristics, and associated risks.

How is Gallbladder Inflammation (Cholecystitis) Treated?

Clinical findings, laboratory tests and imaging are evaluated together in diagnosis. The Tokyo Guidelines 2018 (TG18) use a combination of “local inflammatory findings (RUQ pain/Murphy), systemic inflammation (fever, CRP, leukocytosis), and support by imaging” as the basis for diagnosis; they also classify disease severity. The basis of treatment is early laparoscopic cholecystectomy; guidelines state that surgery within the first hours to days after diagnosis in appropriate patients reduces complications and readmissions. Supportive treatments include intravenous fluids, pain control, broad-spectrum antibiotics necessary fpr patients, and discontinuation of oral intake. Percutaneous cholecystostomy (temporary drainage) can be applied as a bridge treatment in selected patients with high surgical risk or sepsis; in cases of suspected choledochal stones, confirmation with EUS/MRCP and endoscopic treatment (ERCP) are considered.
